When you factor in the timing and the fact that it seems pretty apparent in some of the footage that Andy wasn't going for his neck I would venture to guess that the dudes babysitting the stage just got overzealous. 

Little known fact: Security Guards/Bouncers account for 83% of the global penis enlargement paraphernalia market and are the second largest consumers of homosexual pornography.  The highest consuming being homosexual men.  Not saying there is anything wrong with that.
