Title: Origin of "Put him in the sky"?

I keep seeing this come up, and I know it originated with TK.  Was it from a particular video or interview?  And if so, does anyone have any type of a link?

TK did a pudding pops video, there's a cartoon pudding superhero that shows up and skates with him. you can find it on youtube. He yells "pudding in the sky" but it got translated to Slap as "put him in the sky" and the rest is history.

TK mistakenly thought Lee Trevino had passed away and issued a rather sweet farewell message, stating that “[Lee] will be putting up in the sky”.  It got misquoted as “Put him in the sky” and the rest is history.

There was an interview with TK, when he was a plumber. He got asked if he gets recognized on the job and his response was "No. Plumbin in disguise." Everyone on SLAP thought he said "put him in the sky" and the rest is history

There was a day in the life with TK where he was eating at a burger place and he said "I'll have salt on my fries". Then it got translated to Slap and everyone thought he said "put him in the sky" and the rest is history

it was on his nineclub, he was looking at crob and that massive gross thing on his eyelid and said "chris why you put up with that stye?" but what slap heard was "crob ill put you in the sky" and well the rest is history.
