It looks bad, but the thing about going to a doctor is that they will do 2-3 things:
1) x-ray it to check if its broken (its usually not)
2) Tell you to Rest Ice Compress and Elevate it (RICE), slowly work it out by writing your name in the air with you big toe, and don't put too much stress on it until well after it seems healed
3) Maybe prescribe some vicodin, but having no insurance, that too will cost you, and honestly, vicodin ain't even that great of a high and leaves you constipated as a motherfucker, just go with extra large doses of Ibuprofen (which he also may prescribe horse pills of for swelling) and some weed and you'll be in better shape then vicodin can do.
Basically, wait it out. The big check you can do at home is to put pressure on the side of your shin, if that hurts, you may have broken something, and also pay close attention to how the bone on the outside of your foot (which eventually connects to the pinky toe) feels. Those two spots are where a lot of ankle breaks from sprains occur.
